Arizona notarial acts
A jurat records that a signer signed and swore or affirmed before the notarial officer. An acknowledgment records a different notarial act. Arizona publishes sufficient short-form wording for these and other certificates under A.R.S. § 41-265.

Before an appointment
The document should be complete. For a jurat, the signer signs in the notary's presence and takes an oath or affirmation. The notary completes the certificate as part of the notarial act. If a document does not specify the needed act, the signer should obtain direction from the document's recipient or a qualified adviser.
