Complete packets
The packet combines planning information and the main advance directives. Choose the DNR printing option that works for you.
The orange DNR version must be printed in color. The white-background DNR must be printed on orange paper. The DNR also requires a physician signature.
Mental Health Care Power of Attorney
Names someone to make mental health care decisions if you cannot make those decisions yourself.
Living Will
Records the end-of-life medical treatments you do or do not want and helps communicate your wishes to your health care agent.
Prehospital Medical Care Directive (DNR)
Informs emergency responders about a decision not to receive resuscitation before reaching a hospital. Discuss this form with your doctor, who must sign it. The directive must be on orange paper: print the orange version in color or the white version on orange paper.
POLST
A medical order intended for people who are seriously ill or frail, generally toward the end of life. Discuss whether it fits your situation with your clinician.
Arizona Healthcare Directives Registry Agreement
Used when you choose to register completed directives with the Arizona Healthcare Directives Registry. Registration is separate from preparing the directives.
Online and fillable options
The Attorney General offers an online request to have a packet mailed to you. AzHDR also offers a fillable short-form Living Will and Health Care Power of Attorney PDF. It is a separate form from the Attorney General packet.
