Tell us where to meet

Include your city or neighborhood, preferred date and time, and whether the appointment is at a hospital, care facility, or another location with access requirements.

Describe the signing

Share the general document type and number of signers. If witnesses are required, mention that when requesting the appointment so arrangements can be discussed.

Bring identification and paperwork

Each signer should have acceptable identification and the document ready for the required notarial act. Avoid signing ahead of time when the form requires signing in the notary’s presence.
